[Doctor Rhee Sewhan's Column] Breast Surgery Q & A with Doctor Rhee Sewhan

Hello, I am Dr. Rhee Sewhan, the Head Doctor of Grand Plastic Surgery.

Q1. What is ideal implant size (cc) for natural breast augmentation?
When choosing implant size, you have to take into consideration your height, body type, location of the breasts and size of rib cage, not just the size of the implant in cc.
As you can see, there are devices that can be tried on to see which size implant may be suitable for you.
It is advisable to get surgery after you figure out what size implant would achieve which ideal shape and size.
Q2. What are some types of breast implants?
Breast implants can be divided into saline implant and cohesive gel implant.
Nowadays, we rarely use saline implant.
And cohesive gel implant is used most of the times.
Among Cohesive Gel implants, there are textured type where the surface of the implant is rough and smooth type where the surface is smooth.
Among textured-type, there are round-shaped and teardrop-shaped implant.
Generally, teardrop shaped implant is in textured type surface.
If you look at each implant’s pros and cons, as for smooth type, the surface is smooth and it feels softer to touch.
However, capsule surrounding the implant is more likely to form.
So there is slightly higher chance of capsular contracture.
Therefore, textured type is more commonly used than the smooth type nowadays.
As for textured type, it has the pro of extremely low chance of capsular contracture.
However, it may feel harder to touch.
Therefore, those who have very thin body type may be better off with smooth type.
Otherwise, texture type implant is more commonly used.


 
Q3. Depending on the breast shape, there are round-shaped and teardrop-shaped implant. How are they different?
Circular or round shaped implant really have round shape.
Teardrop-shaped implant however,as you see here, has more protrusion on the lower part. So the teardrop-shaped implant takes on this shape.
Round type implant is suitable for sunken upper breast
and commonly used for those who lack on the upper breast after breast feeding.           Also, if the breasts are located slightly lower than the average, using round type implant would be more advisable.
On the other hand, if the breasts are higher up and if the lower breast volume is not sufficient, teardrop-shaped implant would help to achieve more natural looking breasts.

Q4. When getting breast surgery, which incision would be suitable for me?                                  
Generally, most common incision method are armpit incision and under breast crease incision.
As for armpit incision, the scar can be hidden on the armpit region, leaving no scar on the breasts. This incision method is often more preferred by young women.
Under breast crease incision method is preferred by older women because they already have under breast crease and incision line may be hidden under the line.
If the areola is too big or the breasts look too droopy,
large areola can be reduce and surgery can be performed through areola incision.
 
Q5. How long should I wear supportive garment after the breast surgery?
After the surgery, supportive garment is usually worn until 1 month after the surgery.
Depending on how much space is made and the implant is settled into place,
the garment may be worn until 3 months after the surgery.
For revision surgery cases, since we have to close up existing inner space, the supportive garment may be worn longer than the above.
Generally, for the primary breast surgery, the supportive garment is worn
up until 1 month after the surgery.

Q6. What are some things to be careful of after breast surgery?
Bleeding inside after the surgery increases the chance of capsular contracture.
Therefore, first 1 week is critical for recovery.
Initially after the surgery, it is important to get bandaged on the breast
and minimize arm movement to prevent blood forming inside.
Light exercise or jogging may be done 3 ~ 4 weeks after the surgery. As for any exercise that uses arms should be avoided until 3 months after the surgery.
